Application for Italian citizenship recognition.
Those who wish to submit an application for citizenship by descent (iure sanguinis) must submit the following documents:

Your Italian parent’s or grandparent’s birth certificate (estratto dell’atto di nascita) issued within the last six months, by the Italian comune where the birth was registered.

Birth certificates in long form, duly legalized and translated into Italian.

Marriage certificate of the Italian ancestor. If the marriage occurred in Italy, please submit the estratto dell’atto di matrimonioissued within the last six months, by the Italian comune where the marriage was registered.

Marriage certificates in long form, duly legalized and translated into Italian of every descendant in line, including the applicant;

Death certificates in long form, duly legalized and translated into Italian of every descendant in line (if applicable);

Appropriate documentation issued by the foreign local authority which must certify that the Italian born ancestor who emigrated
never naturalized or acquired any other citizenship abroad.

You may also need a "estratto di nascita in formato internazionale" for that person’s spouse, if he or she was born in Italy.
Estratto di nascita is not to be confused with a certificato di nascita, which does not contain the necessary amount of information.

You will require an estratto di matrimonio (marriage certificate) if their marriage occurred in Italy.
You may need a death certificate (estratto di morte) if your ancestor returned to Italy and died there.

The main differences between these five Italian terms

DOCUMENT
The broadest and most generic term of all. A document can be a sheet of paper, a written record, a digital file, a letter, a signed or dedicated photograph, a passport, or a contract. In Italy, all of these are commonly referred to as documents.

RECORD (ATTO)
A record is an official document created by a public official, such as a Civil Registrar, a Mayor (or their delegate), a notary, or a judge, and then legally registered. The original version is typically preserved in the Municipality's registers, the Prefecture, the State Archives, or the notarial archives.

REGISTER
A register is the bound volume that contains official records, organized by year and by category, such as births, marriages, and deaths. In other words, it is a chronological collection of officially preserved records.

EXTRACT
An extract is the official summary of a record. It is usually issued by the Municipality's Civil Registry Office and includes the essential information, such as the person's first and last name, the date, and the place of birth, marriage, or death.

CERTIFICATE
A certificate officially confirms a person's legal or administrative status based on information held in public archives—not only Civil Registry records.
