The short-form death certificate (certificato di morte) is the document issued by the authority in the relevant city, in which it is attested that a citizen has died.
In addition to confirming the death, the short-form death certificate also contains details on the deceased: first name, surname and date and place of death.

Estratto per riassunto dell'atto di morte

In contrast to the short-form death certificate, the long-form death certificate (estratto per riassunto dell'atto di morte) also contains the time of death and usually the residence of the deceased, as well as whether he or she was married or widowed or other information. It is issued by the authority of the city where the death took place. The certificate is also known as the Death Extract (Estratto di morte), see below.

Integrale atto di morte

The full copy of the record of death (copia integrale dell'atto di morte) is the photocopy of the original record (see document above and below) that contains all of the details appearing in the register of the vital records office.

The full copy of the record of death is issued by the authority in the city where the death took place, and only upon the motivated request of those who have, and can demonstrate that they have, a personal interest, in order to protect a legally relevant situation.

Estratto di morte desunto

The long-form certificate extracted from the register of records of death certifies the death of the individual on the basis of the relevant record of death.

The term desunto is normally used when deaths that have taken place outside Italy are transcribed into the Italian system.
